Our community nursing care team provides education and support to you and your family to become fully independent with managing your health and wellbeing while having in mind that that you will never be alone when you need support from our experienced Registered Nurses. We will assist you in managing your medication, wound care, daily or weekly observation of your vital signs and any other nursing care related tasks. Our Registered nurse will liaise with other health care professionals including your General Practitioner if deemed medically necessary. Our Community Nursing care team will deliver the following;

• Assessment by registered nurse at your own home
• Setting up care plan based on your healthcare needs
• Medication administration education and supervision
• Wound care assessment and care plan
• Stoma care assessment and care plan
• Respiratory support, if you are on Ventilator, BiPAP or CPAP, we got you covered by our critical care registered nurses.
• Continence education, management.
• Palliative support, we understand your feelings, we will be there to support in every step of palliative stages.
• If you are hospitalized for any reason and you are to have carer, we will be there.
